Insurance Information:
We currently participate with the following insurance carriers:

Connecticare (some plans require referrals prior to visit)
United Healthcare
*** HMC PPO – this includes a wide range of packages, please check with your plan to see if we are participating
ENTITLEMENT PATIENTS—We currently provide services for and supervise the Pediatric Otolaryngology Resident Clinic at Yale New Haven Children’s Hospital. Therefore we provide services to all patients within that clinic.
All other insurances—If we do not accept your insurance, and you would still like to receive services from our group, we would be happy to work with you in respect to private payment.
We make every effort to coordinate
payment for your visit with your insurance carrier. It is your responsibility,
however, to provide us with accurate, up to date information which will assist
in the collection process. All
claims that are denied by the insurance carrier are the responsibility of the
patient/guardian. If your insurance
plan requires a referral from the Primary Care Physician, please bring the
referral with you or have your doctor call us or fax a copy prior to your visit. Your visit will be rescheduled if the
referral is not on file at the time of your appointment.
